STEP 2	Fill out the form. Please note, if the name meaning for your child is not available, we will strive to replace it with their middle name (if they have one) or you can let us know what you'd like instead. Horoscope, Chinese zodiac and special message are all optional. If you choose to add a photo, the higher the resolution, the better the print. Also, the photo should be in a portrait format.
